Dear all, we have a patient with the following history.
Known Hyperthyroid on block and replace for 1 year before stopping for re-evaluation. After 6 months the results were
TSH – 2.19mU/L (0.27-4.2)
FT3 – 4.8pmol/L (3.95-6.8)
FT4 – 6.5 pmol/L (12-22)
The TFTs then recovered to normal before the patient relapsed again. Pt does not want RI or surgery and was put on carbimazole again. After 6mnths this was titrated
down to 5mg and gave the following results
TSH – 2.64mU/L (0.27-4.2)
FT3 – 4.2pmol/L (3.95-6.8)
FT4 – 6.8 pmol/L (12-22)
Patient is clinically euthyroid. Does anyone have any comments on the disproportionately low FT4 and what may be causing this?
