Dear List, Following my query yesterday about the Berlin Questionnaire, I've had some very helpful responses. 2 people kindly confirmed that the Berlin Questionnaire isn't available on the Web but that it may be available from the corresponding author in the original study on request (http://www.ncbi.nlm.nih.gov/pmc/articles/PMC137813/?tool=pubmed). The author doesn't want the questionnaire to be forwarded to multiple recipients as this may invalidate its use as an instrument.
