I just got a reply from Book on Demand: "Hi Jeff, thank you very much for your email, we provide self publishing service and sometimes, very rarely some people upload works that don't belong to them. I will make sure the book will be off sale ASAP, and I will write a letter requesting to block the account of the person who uploaded your book."
