I agree with this cautioun Diana. I can count on one hand the amount of times I have disclosed actual diagnosis and don't believe that information is needed for management to understand why adjustments are needed. IMHO they need to know/understand the symptoms impact on function e.g pain affecting concentration causing a slower work pace and increased need to recheck work for errors.

I believe that she also advises caution in disclosing medical information in the report  even if it is in the public domain (eg on sick notes) without the written consent of the patient.
