Is this not a case of you say tomato and I say tomato? What is more important is every one is clear if restrictions/adjustments/accommodations are permanent or time limited.
I also prefer the term return to work plan instead of rehabilitation plan as it keeps the target firmly in focus and does not have medical connotations like ' rehabilitation'.
