The ACP just came out last week with an updated guideline for VTE prophylaxis in medical (and stroke) patients.

The first recommendation is: "ACP recommends assessment of the risk for thromboembolism and bleeding in medical (including stroke) patients prior to initiation of prophylaxis of venous thromboembolism"
 
This certainly makes sense, BUT they do not suggest any particular tools for risk assessment.
 
So, my question for the group is, does anyone know of any VALIDATED tools for prospectively assessing a patient's risk to develop VTE/DVT while in hospital? (NOT tools for predicting DVT likelihood in symptomatic patients, eg the Wells rule, but for assessing patients on hospital admit as to the risk of developing VTE in order to decide how necessary prophylaxis is?)
 
I've tried searching, and not found anything - not sure if ther eis nothing out there, or I am just looking in the wrong places...
