I think that this sounds like a precursor to some form of capability review, and possibly dismissal if a return to work isn't likely within the foreseeable future - and the manager is setting the foundations for this . Is this the private of public sector? I only ask because in the public sector managers are being encouraged to take a more robust approach to absence management issues
 
Fact is that without a crystal ball you cant tell how the manager has all this information - it may be as simple as the fact that she has "asked around" his colleagues, some of whom I dare say he has had contact with (is there a contact policy within his company? It may be that there are notes of telephone conversations etc)
 
I don't think she is holding a medical review - it looks like she is determining what the likihood is for a return to work, and in what capacity. If this looks unlikely they will probably start whatever procedure they have for determining capability

I would like some advice on a situation someone I know is in.
He has been off sick for more than 12 months due to a serious illness. (Ca)  His GP has being sending Sick notes to his employer.   He is now on leave without pay. His work place has never asked for a medical report from his GP.  Recently he got this letter from his manager vividly describing all his illness, type of interventions he has had and so on.  She (manager) is writing to have a review meeting in the home of the sick man "to establish how you are and what progress you are making towards recovery and also to determine if there is anything I or the company can do to help you in your return to work.
 
She further went on to give the agenda of the meeting as:
 
1. Your medical condition and the prognosis for the future.  Discussion of the medical report (if any)
 
2.  Consideration of job adjustments to rectify the issue.
 
3.  Consideration of alternative employment/job roles.
 
4.  The way forward/ ongoing review.
 
My questions are- how did she get all these information about the type of cancer, what surgery etc?   Is she the right person to hold a medical review meeting with the employee?  Why no involvement of an OH practitioner or doctor?
 
Please let me your view on this.

Of course but I would advise  careful wording of your report request to
maximise the chance of getting a report which is of any use to you. Ask
specific questions which relate to the information you wish to acquire.
There may be a range of information which you need - and you would need to
formulate your Q's to get that information.

With regards to a prior history of back problems: Something along the lines
of:

" Mr X was injured in a fall at work on.......
A return to work recovery programme has been devised but with little success
to date. (you could give a few details here if appropriate) It would be
helpful to have further information regarding any current or past health
problems  Mr X has experienced which may be impeding a successful return to
work. Any information you can provide which can be used to assist us in
reviewing and thus optimising the success of this programe would be
appreciated. ......"
