writing to GP
Of course but I would advise careful wording
of your report request to
maximise the chance of getting a report which is of
any use to you. Ask
specific questions which relate to the information you
wish to acquire.
There may be a range of information which you need - and you
would need to
formulate your Q's to get that information.
With regards
to a prior history of back problems: Something along the lines
of:
"
Mr X was injured in a fall at work on.......
A return to work recovery
programme has been devised but with little success
to date. (you could give a
few details here if appropriate) It would be
helpful to have further
information regarding any current or past health
problems Mr X has
experienced which may be impeding a successful return to
work. Any
information you can provide which can be used to assist us in
reviewing and
thus optimising the success of this programe would be
appreciated.
......"
I hope this is helpful
