It is rare that physio notes are of use to anyone but physios due to the abbreviations and coding that are used. We always recommend a report so the diagnosis, treatment, rehab and prognosis is in long hand so all can understand it.

Why would you need the physio notes? Wouldn't the physio gain consent to write you a report the way that we do??  What does the Charted institute say?
