Hi,We give the employee the option to see the report before it is sent to HR so that they can approve what we have written (via e-mail). Once we receive their approval we forward a copy of the report to HR.

How do OHA's who carry out telephone consultations cover themselves regarding writing reports following consultations. With face to face consultations we have employees sign a consent form which details the information which will be disclosed in the report. With telephone consultations we are unable to do this. the employees sign a referral form stating they are willing for the consultation to take place and for a report to be issued. at the end of the consultation I always discuss with the employee what will be written, however if they disagree its my word against theirs. obviously we have written notes. 
 
I would like to know how other individuals deal with this situtation. My company are considering recording all calls for my benefit, however I have concerns whether some employees may have concerns with there personal/medical information being disclosed.
