Reminiscence prompt cards

Hello everyone,

I'm currently meeting with local groups to sort out our reminiscence boxes for use with older peoples groups, people with dementia, care homes etc.

Has anyone got an example of a prompt card they use in their boxes?

I'm after something short and easy to use that will help carers coax a conversation from the users.
The boxes will be going out for use by carers who won't have seen them before, so nothing too long or complicated. Maybe just a bulleted list of questions?

Any ideas / examples / general advice gratefully received!
