We've had no problems -but see attached field safety notice.
IL have responded by giving NHS GG&C IL Gem 3000s with potassium only for those areas that require it.(disabling the K+ on the GEM 4000s)

An e-mail was aslo rec'd on 13/06 as follows -

please fine the latest update with regards K+ on GEM 4000.  Two areas I will cover here software and root cause.
Software
   Version 2.1.4 went into 7 US sites with VPN connection to R and D Bedford Friday, 10th June. By Thursday, 16th June a decision will be made as to a limited release. All being well the software will be with the UK team week beginning 20th June.  The software is a patch and there is no server upgrade. The software is very conservative and will disable the K+ sensor before low result can be produced.  With the in house test we have experienced a 50% failure of K+ due to the software patch. Our suggestion for those sites that want K+ on the GEM 4000 is to leave GEM 3000 on site, consequently any failure will not mean additional pack changes. The software is a short term fix the long term is root cause identification and correction.
Root Cause -
R and D can now reproduce the issue and are close to root cause of failure. The outer K+ membrane, over time, becomes cloudy.  A crystal structure is forming a film over the membrane, this causes a resistance layer and subsequent slow response. Investigations are now focused on the five components that make up the outer K+ membrane.
Forecast is now for root cause resolution by August this year.
