Re: requesting OH records...  I recent wrote a policy for our dept as we didn't have one, or a set procedure or forms.  It's yet to get information governance approval, but if anyone wants to critique it, please let me know?

Surely the legal position under the Data Protection Act is they the information is the patient's and if they request a copy of their record, they get it (with a few caveats about information harming the patient etc).

What they do with it and who they share it with is up to them.

Quite a different thing offering to give an opinion...
