Errr... yes she had, the consent forms always say all of my medical records. Just because her lawyers had not told her the implications of this.

We sometimes phone the patient to check they fully understand.

I  gather that you really cannot charge more than UKP50 but you can ask for it up front and then still have 40 days to provide it. There may be scope for a fee for expediting this...

There is no obligation to provide it on paper or to post it.

However 3rd party references are a problem. A patient of ours complained bitterly because the husband's solicitors used a referral letter to his psychiatrist discussing her behaviour ( marital breakdown was relevant to his depression) in court against her. She had not given consent for that to be released.

I am not even sure that it has to be on paper. If you have the notes 
scanned already a CD would do.
