I am trying to
create an feedback form to send to managers for their evaluation on the OH
service and on our report content.
If there is
anyone willing to share their evaluation form (specifically to
managers) to give me some additional inspiration that would be much
appreciated. (I have created one for employee feedback but want to also
assess what our managers view of the OH service is). Assistance accepted on or
off list. (addresses below)
