In my previous (NHS) post we used these from Epinet..cut & paste link to access
http://www.healthsystem.virginia.edu/internet/epinet/forms/epinet3.cfm

They were admittedly time consuming but you could adapt the questions to your own needs

In my Occ Health dept we are due to start operating a Needle stick injury hot line and we are preparing our flow charts regarding the questions we will be ask
ing. Does anyone have any examples of this that they are already using?
