Rita nailed it on the head.  The best way to avoid the payment for Consultant 
repport  (however small) is to ask for a GP report.  Almost all of them include 
a copy of the Consultant report.  Any procedures out on their patients get 
reported to them.  Just add the adendrum that Rita suggested.  It always worked 
for me.
