The blog is dedicated to keeping alive the memory of significant records management incidents or situations. In each of these situations inadequate systems and poor practices contributed to the outcome. Not all the reports are recent – but all document situations where individuals or organizations were exposed to significant personal or business risk. Several of these incidents resulted in loss of life. Initially the blog was published as part of the Synercon Web site, Lest we Forget has recently been converted into blog format to facilitate tagging, comments and to make maintenance and archiving easier. We welcome your input into our new site!

The Tribunal Appeal between B Francis, the Information Commissioner and South Essex Partnership Foundation NHS Trust should give some food for thought. Does not touch on keeping records longer than necessary, but does highlight the risk of not knowing where your records are, who owns them and who can have access to them. Dear all
I am currently writing a paper to present to senior managers which emphasies the importance of our review and disposal procedures.
While we are all familiar with the Enron/Sarbanes Oxley scandals I am keen to find other real world examples from the UK private sector where a business has been placed at unnecessary risk by keeping their records longer than needed.
If anyone can suggest possible sources for such examples or have any they are willing to share I would be be most grateful.
