Why do we try and report  "Estimate of what the total calcium would be if the albumin were something else" rather than "Estimate of ionised calcium". Is it only for historical reasons?

"Estimate of ionised calcium" woud be more consistent with pathophysiological understanding and with results from blood gas analysers.
