Greetings from Bangladesh, where we are having a regional workshop on strengthening midwifery in South Asia.

I have a concern, very close to my heart as a promoter of Midwifery: I would like to accumulate as much evidence as possible on the positive role that Midwives play (or have played) in the reduction of maternal mortality in low-income countries, and in mid-income countries. So far I have only that famous graph showing a correlation, but I am after more detailed country examples (or district examples). In particular (and this is tricky, but useful in our advocacy vis a vis India), I wish to find evidence that "pure" midwives (direct entry course of 3 years with a lot of practice) have done better than "nurse-midwives" or "auxiliary nurse-midwives" (mixed course on nursing with a little bit of midwifery). India is interested in the experience of "mid-level income countries" and "emerging countries" such as Turkey, Iran, Jordan, Brazil, Thailand, Malaysia, Mexico, Chile, Eastern European countries, etc. What advice could you give us, based on your experience, and what contacts, or references, could you indicate?

I also announce that we are organizing a Symposium on Strengthening Midwifery during 2 days just before Women Deliver in Washington (5-6 June).We  aim at inviting 200 prominent midwives and advocates and researchers all concerned with the role that midwives can play in reducing maternal mortality (and promoting MDG5 B). Do not hesitate in proposing participants!

I am not sure how helpful our Cochrane review on midwife-led care will be to you as none of the trials were done in low income countries. What it does show is women who receive midwife-led models of care compared to other models of care (shared care and obstetric led care) have significantly higher rate of vaginal birth, breastfeeding, control and reduced interventions such as pain relief, instrumental delivery, episiotomy.
Numbers are too small even in a meta-analysis of 12,000 women to look at maternal mortality, but women who receive midwife-led care have a significantly lower fetal loss before 24 weeks gestation.
