A 37year old fit runner taking no extra protein/supplements/losalt/bananas/antacids (he is only on fluoxetine) has a persistently high potassium that is unexplained. Blood count normal (temporary high platelets on one occasion which returned to normal). GP wants to know why and is it OK for him to keep on marathon training? To settle the matter I asked the patient to attend the hospital for a fresh Li heparin sample (no other sample tubes) centrifuged promptly with no temperature change and prompt analysis. Plasma sodium 142 mmol/L 135 - 146 Plasma potassium * 5.2 mmol/L 3.2 - 5.1 * high Plasma chloride 105 mmol/L 96 - 108 Plasma bicarbonate * 31.4 mmol/L 22 - 29 * high Plasma creatinine 81 umol/L 62 - 106 Any suggestions - what gives a high bicarb and high potassium? regards Andrew Iversen This electronic message contains information from Brighton and Sussex University Hospitals NHS Trust, which may be privileged or confidential.
