Hello given the length of absence, 'no pay' situation, and that they've not engaged with OH then this is now something HR need to handle surely? I personally wouldn't write to the GP or anyone, as you've previously attempted but had no response, so pass it to HR to deal with, it's not an OH issue any longer.
