I agree!

It is my understanding that completed consent form must only be used for the purpose it was obtained (i.e. the consent is informed), is valid only the once and should be used within an agreed timeframe (e.g. on receipt or following your next appointment with your Dr on the 19th of January , I intend to write to your GP/Specialist ... outline/draft of letter to Dr enclosed.
