Hi Jeba, That is a very interesting question. My suggestion would be, Why not just do an academic PhD, in any appropriate department, with your subject matter and or sample frame based around midwives? Of course this will be all about generating your own theoretical contribution to the academy rather than contact time but I would have thought, given your circumstances, that would be perfect! Health studies research are quite well funded (provided, of course, you apply through a department that has a good research credentials and secure a prolific supervisor) moreover, loads of PhD students all over the U.K. read part time and or conduct their research as distant students. Not quite what you were looking for, vocational PhDs seem to be quite a different thing to the traditional idea of making a new and unique contribution, but it might help you.
