You need to get the following patch applied to your installation of WebPA http://sourceforge.net/tracker/?func=detail&aid=2504236&group_id=194016&atid=947739
 
This should fix the problems you are experiencing.

Subject: Form Input Broken - Any Thoughts???

Hi WebPA Community

Ive seem to have cracked my pervious WebPA nut (modules and users). Can anyone help by suggesting a course of action for:

My Forms > Add a New Criterion

- It will only currently allow me to input one criterion

- When I click on the "add a criterion" it opens the single one already inputed

- Also the "import form" throws out an blank error message

Help!
