Religious views on 'Do Not Resuscitate'

Dear all,

I wanted to ask if anyone has compiled the views of the various religions on the issues that come out of the 'Do Not Resuscitate' policy. What is the view of various religions on someone agreeing that they should not be revived if the quality of life after resuscitation is judged to be extremely poor?
