So far as I am aware the situation is as follows:

 

1)       Serum does not count as a tissue, and therefore no HTA license is required for long term storage.

 

2)     You will require ethics committee approval and research governance approval.  If long term storage of serum is desired, with potential future use for multiple purposes, then the best process may be to seek NRES approval to establish a research tissue bank (a process which can also be used to establish a serum bank).  You can get guidance on this procedure at www.nres.npsa.nhs.uk/EasySiteWeb/GatewayLink.aspx?alId=4792
