We discussed the topic of religious names at some length under the heading New Names in Religion 6-9 May 2008 (the archive can be found through the website listed below). The conclusion was that, despite various exceptions, this is a fairly recent custom, which becomes widespread only with the religious reform movements of the 16th century and later.
