We do not routinely request GP reports but all applicants are asked to supply a completed form from their GP which asks a short number of yes/no questions

If you pre employment screen student nurses please can you contact me, I want to find out how many of you routinely request a GP report for applicants to nurse training. I am writing an assignment on this part of our current practice to see if changes needed or appropriate.
