Following on from other responses. To get round the issue of asking GP's/Consultants etc for their consent to release a copy of their reports, I picked up this commentfrom jiscmail and now use all the time.
 
When sending a request, a statement is put in to the effect of -'If a copy these notes is requested, your report will form part of the copy unless you state otherwise in your report'
