I think one has to accept that there aren't enough OH services. Locally to me, I can only think of only 3 employers with an OH service, and I work in one of those. What does a GP do in his/her 10 minute appointment when the patient's physical/mental health problem needs to be managed and there is minimal information about what the job can involve or what RTW arrangements there might be available? Usually, the patient has no idea of any return to work program at work, which is probably because there isn't one. The employers don't bother to contact the GP or the patient about reduced hours or reduced workload, they just want the body back at work. Or the DSS office sends the patient up here to demand a Med 3 ( even when they are fit, just to reduce the numbers on Jobseekers allowance) or sometimes even to demand a DS 1500 ( the form used when the life expectancy is 6 months or less). In the past, I've had to go through the DS1500 form line by line with the patient to show them that the DSS office has got it wrong.

See http://nhsblogdoc.blogspot.com/ for a more rabid view of what happens when the DSS office and the GP disagree
