Good afternoon list.
We are presently revamping our NHS Trust pre emp questionnaire for healthcare workers, and rather than as at present confront the applicant with a barrage of over twenty questions pertaining to mental and physical ability/history, would like to encompass same in just a few questions. Questions regarding immunisation and health surveillance requirements being dealt with in a separate section.
Any ideas or templates you'd be willing to share?
