As med3s are really about claiming Statutory Sick pay, altering the form would be fraudulent claiming of state benefits , or "benefit fraud"  and there are anonymous telphone lines for reporting  this. Or HR might be willing to do this formally and openly.
 
For small organisations without OH physicians it is possible to ask HM Revenue & Customs to investigate ill health which is causing absence from work if it has been going on for a long time.
