Hello list
As med3s are really about claiming Statutory Sick pay, altering the form would be fraudulent claiming of state benefits , or "benefit fraud" and there are anonymous telphone lines for reporting this. Or HR might be willing to do this formally and openly.

For small organisations without OH physicians it is possible to ask HM Revenue & Customs to investigate ill health which is causing absence from work if it has been going on for a long time.

See:

When and what medical evidence should your employee give you?

Evidence of incapacity for work
Managing sick absence
Seeking medical advice about lengthy absences
Seeking medical advice about frequent absences for short periods
Action when you get the medical advice
Employee refuses to give consent
Control periods, common illnesses and abbreviations
"For example, using the group 'mouth and throat disorders', if the doctor's diagnosis is tonsillitis it would be reasonable to seek advice after four weeks. If you wish to ask HM Revenue & Customs to get a Medical Services opinion, you should not do so earlier than four weeks after the sickness started. "

Cheers
Diane
