It sounds very suspect that he can't rem his meds, I would ask him to bring in the meds.
If he refuses to bring in the meds that really is a concern.
About the consent I would inform him politely that I would need to inform HR that he is not willling to give his consent (ensure he knows the request for report is just to do with his alleged crohns disease) and without the OH Interpretation of  the gp report they will act on their limited information and this may not be in his best interest. 
If he is genuine, once you explain that you are working to help management understand his illness and give him examples of how you could help him he should jump at the chance to get your help.
He may just be suspicious and worry (wrongly!!) because of the food hygiene regs concerns about diarrhoea (we know its infectious diseases that are the concern but others don't always)

Hi everyone
I have  guy who won;t consent to me contacting his GP, he states he has 
crohns disease and 'can't remember' his medication.
He proides sick notes from his GP but they never say crohns.

There is suspision that he has altered one of his sick notes is there any 
way of verifying this?
