We admitted an 81 year old chap last night with 

Na = 155, K = 7.8, Urea = 99.6 mmol/L, Creatinine = 1694 umol/L, Hb = 5.9
g/dL 

Clinical details "epistaxis" (!!), long-term warfarin patient whose U&Es
were normal when last done in Oct last year. 

Is this a record?
