Perhaps the considered view of Diana Kloss would be of help. She quotes the Faculty of Occupational Medicine  Guidance on Ethics 1993 (p 62 3rd Edition OH Law)
 
"Advice given to management about results of a medical assessment should generally be confined to advice on ability  and limitations of function. Clinical details should be excluded and even  when the individual has himself given clinical information to management, the OH physician should exercise caution before confirming any of it. If a report on the health and fitness for work of an employee is to be communicated to management it is important to ensure that the employee understands the physician's duty to the employer. The contents of the proposed report should be discussed with the employee and consideration should be given to obtaining the signed consent of the employee to be examined in such circumstances"
 
My favoured action is to get signed consent for the report but not to create the report while they are there. I later send a copy of the report to the employee.
