Hi,
 
Thunder is a good quality free screenreader:

There are loads and loads of free text to speech thingys, the one I like the most is Readplease 2003
 
http://www.screenreader.net/
 
These are both basic packages with the possibility of upgrading to the 'full' versions. However as their free they won't suddenly lock you out after 30 days, like shareware.
