Subject: CSF lactate for detecting mitochondrial disease and or plasma lactate

A few questions to try and determine what is the practice, and what is the evidence for the practice.
1. Does CSF lactate aid in the detection of mitochondrial disease/disorders, and do laboratories offer it routinely?. Yes but may be normal.
2. Is it better than plasma lactate, or do laboratories measure both. CSF may be raised when plasma N due to hepatic clearance
3. Is there insufficient evidence to support the measurement of both CSF and plasma lactate, or of either alone?  Evidence for both.
4. If you do measure lactate, do you measure it in the laboratory, as the blood gas analysers are not certified for CSF?  Laboratory.

Thanks in anticipation for the help,
Helen
