Would be grateful if anyone on the list would be wiling to share experiences and thoughts on Case Management i.e.
Definition - what is case management
What does it include - and not include?
What experience have the list members had with liaising with other care providers when involved in managing a case?
What have been some of the pitfalls and successes? Are other health professional cooperative or resistant to an OHN case managing?
Any good articles that can be recommended would also be very helpful
