Depends on whether the assessment report records sight of the evidence, indicating who the original evidence came from, and the contents of the report. A well written report should indicate the evidence received. If there isn't enough info in the report, then we would request sight of further evience. To clarify, we would not expect students to provide medical evidence when they first approached us. However, once a student is asking for exam adjustments / adapted accommodation or any of the other adjustments we might put in place we ask for evidence to be provided. We are looking at this whole area at the moment and I thought it would be useful to find out how other institutions manage this. Do you have a hard & fast rule about what is acceptable? For example, if a student with a mental health difficulty wanted to access your services are there a range of people you would accept evidence from? What about students with Aspergers / ASD? (BTW we don't do DSA assessments here so students are not providing evidence of disability for those to us. Do other institutions accept DSA assessments as evidence? We have discussed that idea & feel there are some difficulties with it).
