Dear Colleagues,

We use the Sebia system for alkaline phosphatase isoenzymes and recently found an unusual pattern in a patient with an extensive sino nasal carcinoma, with liver metastases. The electrophoresis shows one band corresponding to the slowest intestinal band and another corresponding to the liver 2 or macrohepatic band. There is complete absence of the liver 1 and  bone fractions. Heat stability studies showed that there was no heat stable fraction present. The ALP level is now just within the reference range but the GGT is about twice the upper level of the reference limit.

Has anyone else who uses the Sebia system seen a pattern like this? The pattern with and without lectin is identical. The Sebia technical department has not seen anything like this. If it is macrohepatic and intestinal could someone tell me the significance of this pattern.
