I found that this sites listed lots of sites that provide a comprehensive tools on learning EBM. It may not have a handy questions sheet but I think you can create your own questions based on some of the teaching materials available in here.

I would be very grateful if people could point me at (or recommend) tools to assess EBM skills. I am looking for a tool (perhaps an MCQ) that could be used to assess individual learning needs and plan group training.
