Hi Marlene We did some detailed analysis within our evaluation of one-to-one (caseload) midwifery which suggested that if you need to target at some women, this should be for women who are socially disadvantaged or have more problems with access to care. We also found that midwives who have chosen to work in this way find it very rewarding. We set up partially through Sure Start (2 of our midwives are funded by them, the other 4 by the NHS Trust). This was a new SS initiative which has been funded by PCT, so we went at it from that angle--improving health and reaching SS targets. However, now things are changing and we are getting more women referred to us who have particular need from caseload care who are outside our area (I think every woman should have it, but that's another battle!!). As SS funding finishes, we think we will be primarily caring for more needy women--for example, those with mental health problems, previous traumatic labour/birth, social problems, women who continually fail to keep appointments at GP (usually because they have other small children and find getting out difficult).
