For information and discussion on the development of pastoral care (particularly in England) just before and after the Fourth Lateran Council, you may want to consult:
Thomas of Chobham, Summa confessorum, ed. by F. Bloomfield, Analecta Mediaevalia Namurcensia, 25 (Louvain, 1968) – Bloomfield discusses in his introduction how far such summae were academic works and at what point they percolated down to parish priests
similar concerns (as far as I remember) in Joseph Goering, William de Montibus: The Schools and the Literature of Pastoral Care (Toronot, 1992)
and Leonard Boyle, ‘the Fourth Lateran Council and Manuals of Popular Theology’ in The Popular Literature of Medieval England, ed. by Thomas J. Heffernan, Tennessee Studies in Literature, 28 (University of Tennessee Press, 1985), pp. 30-43 is a very good introduction to this area.
