There is a standard model release form for video work that I have. It covers the standard for video work and for location work. The only thing I can't find it at the moment. I have a Word version somewhere - half built - because the original is on a PDF that's fairly illegible (how anyone is supposed to use it directly I don't know!) If anyone wants it I will ahev to mail it as an attachment - let me know. I'm out now until Monday - so it won't be until then. You'll have to change the UCE stuff for your own institutions.
