Dear All,

We are hoping to pilot a scheme, in tandem with all the other support mechanisms already in place, whereby students who have registered with Disability Services are given a form detailing their support needs (i.e. Notetaker, lecture materials in advance) that they can present to lecturers as and when necessary. It is envisaged that this will be A4 size and include some form of checklist and teaching tips for the lecturers. Does anyone have something like this in place already who would be willing to share their template? If you have something like this in place can you provide any feedback on how it has worked?
