In connection with this I recently was an Information Governance Consultant
reviewing Information Policy at a Primary Care Trust. This included
consulting on an Information Sharing Policy.
I found that Torbay was very active in this area. See

Hello,
Has anybody produced a policy (or know where I can find one) for access to
files by children and young people that they would be willing to share? I
have been tasked with creating one specifically for those children and young
people in residential homes or fostering placements. I have been given a
very short timescale, hence the plead for help!
