I raised this issue some time ago on the mailbase. Impaired fasting glucose, often repeatedly so, but 2 hrs post glucose result is lower. It is relatively common here in SE London. Our PCT guidelines advise to do OGTT on all patients with IFG and also those with normal FG but strong clinical suspicions for DM. We have been through the common causes - non fasting, exercise after glucose load, smoking etc Interestingly this is occasionally seen in patients who have an initial IGT response and are rechecked at 1 - 2 years later after dietary lifestyle intervention. If only we had measured their HbA1cs each time ! Needs a really good audit.
