Essentially the student has to reflect on their grammar, spelling, how they addressed the question, etc., and grade their performance. The tutor also marks the sheet so that a comparison of the tutor's opinion can be made with that of the student. The reverse of the sheet has information on the standard of work getting a I, 2.1, etc, some tick box feedback (saves lecturers writing 'you must improve your referencing' a hundred times), and space for individual feedback. Students are required to hand this sheet in with every assignment, and it is returned to them with the essay. Don't know if this is what you're looking for, but let me know if you'd like a copy.
