Whitaker and Maxwell Johnson, _Documents of the Baptismal Liturgy_, 3rd rev. and expanded ed. (Collegeville, MN: Liturgical Press, 2003). A good secondary survey is J.D.C. Fischer, _Christian Initiation: Baptism in the Medieval West_ (London: SPCK, 1965). The names Stan Metheny mentioned are good leads for more recent work. You might try Maxwell Johnson, _The Rites of Christian Initiation: Their Evolution and Interpretation_ (Collegeville, MN: Liturgical Press, 1999).
